Crystal Page Setup dialog does not have PRINTER button

When printing to Printer and using the Crystal Runtime XI-R2 or XI version. (these versions are installed with recent versions of Forms Printer) - The Crystal "Page Setup" Dialog will be missing the "Printer" button when running on Windows Vista or newer operating systems (Vista, Windows 7, Windows 2008, Windows 2008 R2).

This can be worked around by using the keyboard equivalent Alt-P (even though the button is not present, using the keyboard equivalent will open the dialog to choose a printer)
